The focus target is a new unitframe introduced with TBC.
By targeting a mob/player and typing (or clicking a macro) /focus you will set your focus target and an additional target frame should appear. This window will not change targets until you select a new target and type /focus again. To clear it simply have no target and type /focus.
This is an excellent way to monitor your banish target of the mob you are crowd controlling via fear. Macros below.
Set/Unset focus target
This sets your target as your focus, and clears your focus when you click it with the ctrl held down.
/focus /focus [modifier:ctrl] none
Banish Focus if it exists, otherwise banish targetted mob.
#showtooltip Banish #show Banish /cast [target=focus] Banish; Banish
replace banish with fear to create the same macro that fears your focus.

did they actually add a unit frame for it?
focus was around before BC was released...but it never actually added a visible frame before...it would just add a glow around your target when you had your focus targeted...

Well it was added in the BC code base, so with the 2.0 patch, but that is just semantics. The default UI may just make the focus target glow and expect you to use [target=focus] macros, but I haven't used the default UI in a long, long time. All popular frame replacements have a focus frame ie AG Unitframes
